Job summary

Harbor Healthcare System in Independence, MO is seeking a qualified part-time RN Case Manager for outpatient services. You will work as an integral part of an interdisciplinary care team, coordinating admissions, assessments, discharges, and pronouncements with independence and expertise.

The ideal candidate holds a Missouri RN license, has 1+ year RN experience (Hospice/Home Health preferred), strong communication, and reliable transportation for travel as needed.

Qualifications

Responsibilities

• Admissions coordination and patient/family liaison.
