Job summary

Qualifications

• Missouri RN license is required.
• 1+ year RN experience preferred.
• Hospice or Home Health experience preferred.
• Effective written and verbal communication.
• Ability to travel with reliable transportation.
• Clear background, drug screen, and driving record.

Responsibilities

• Admissions coordination and patient/family liaison.
• Perform assessments and coordinate appropriate care.
• Coordinate discharges and pronouncements as part of care plan.
• Collaborate with interdisciplinary care team in outpatient setting.
