Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for an outpatient role in the Plattsburg area. The RN will admit and coordinate care, perform admissions and assessments, and manage discharges and pronouncements in collaboration with the care team.

Qualifications include Missouri RN licensure and preferred 1+ years of RN experience, with hospice or home health background. Strong communication and travel ability are required; a clean background and driving record are mandatory.

We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager positionin the Plattsburgarea.

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.
