University of Toledo is seeking an RN Case Manager for the Outcome Management Outpatient service line. The role focuses on comprehensive assessment, planning, facilitation, and advocacy to ensure optimal clinical, financial, and patient satisfaction outcomes across the outpatient to post-acute continuum.

The ideal candidate holds a BSN and Ohio RN licensure, with at least 3 years of clinical experience, strong organizational and communication skills, and a valid driver’s license.

Competenze

• BSN with current RN licensure recognized by the State of Ohio is required.
• Minimum of 3 years clinical experience required – preferably in specialty field.
• Possess organizational, problem-solving, and critical thinking skills.
• Demonstrated ability to work independently and effectively under minimal supervision.
• Excellent written, verbal and collaborative group communication skills.
• BLS Certification is required.
• Valid driver’s license and must maintain state minimum auto liability coverage.
