Job summary

Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered hospice care. You will conduct comprehensive assessments, develop and adjust individualized care plans, and work closely with a multidisciplinary team to coordinate services for patients with life-limiting illnesses.

You will participate in IDG meetings, ensure timely documentation, and maintain compliance with Medicare and agency policies. A valid RN license and reliable transportation are required.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate or Baccalaureate).
• Current State Registered Nurse license.
• Current driver's license with reliable transportation and auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical settings.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients and their needs.
• Implement and revise the Plan of Care as needed.
• Coordinate care among patient, caregivers, physicians, and other healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and contribute to care planning.
• Provide on-call coverage as assigned and timely documentation.
• Ensure compliance with local, state, and federal laws and regulations and agency policies.
• Perform other duties as required.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License

• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
