Job summary

Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care for patients with life-limiting illnesses and their families. You will assess, plan, and coordinate care to improve quality of life.

Responsibilities include comprehensive assessments, implementing POC revisions, IDG collaboration, on-call coverage, and timely documentation while ensuring regulatory compliance and high professional standards.

Qualifications

• RN license required.
• Driver's license and reliable transportation.
• Two years of RN experience preferred in healthcare/clinical setting.
• Graduate of an accredited Nursing program (Diploma/ADN/BSN).

Responsibilities

• Conduct initial and ongoing assessments of patients with life-limiting illnesses and needs.
• Implement and revise the plan of care (POC) as necessary.
• Collaborate with patient, caregivers, physicians and other health professionals.
• Participate in Inter-Disciplinary Group (IDG) meetings and care planning.
• Provide on-call coverage and on-call services as assigned.
• Submit documentation timely and accurately.
• Perform other duties and ensure compliance with laws and policies.

What You Must Have

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
