Role Description

This is a full-time in-office role for a RN Case Manager located in the Harrisburg area, PA. The RN case manager will be responsible for conducting initial patient assessments, gathering comprehensive medical histories, assisting in care coordination, and ensuring the smooth intake of new patients. This role will involve both in-office work and some work from home duties. The RN Case Manager will also be responsible for maintaining accurate patient records and providing compassionate support to individuals with I/DD and their families.
