Job summary

Abarca Health LLC is seeking an Associate Case Manager in Puerto Rico to support Medication Therapy Management and Disease Management programs. The role involves direct to-beneficiary contact by phone or in-person visits, coordinating care, and collecting medical histories to assess treatment impact.

You will educate beneficiaries on medication indications, discuss bioequivalents, and collaborate with Health Educators and cross-functional teams to ensure CMS compliance and high-quality service.

Qualifications

• Associate’s or Bachelor’s degree of Science in Nursing (BSN) or higher.
• Current and Active Nursing License.
• 3+ years of work-related experience.
• Knowledge of therapy management concepts (Asthma, Diabetes, Hypertension, CHF, Cholesterol).
• Excellent oral and written communication skills.
• Flexible hybrid work model with required on-site days (Puerto Rico location).
• Availability for on-call hours may be required.

Responsibilities

• Discuss and update medical histories and lifestyle with MTM/DM participants.
• Answer questions regarding medication therapies and bioequivalents.
• Educate beneficiaries on following medication indications.
• Ensure MTM/DM programs add value to life and health.
• Collaborate with Health Educator and cross-functional teams to deliver high-quality service.
• Promote medication safety and CMS compliance.
• Stay informed on advances in drug therapies and chronic disease management.
