Kelly Science & Clinical is seeking Associate Case Managers to support a healthcare technology and pharmacy benefits organization in Puerto Rico. This remote role offers weekly pay and a flexible hybrid model with on-site days as needed.

Responsibilities include Medication Therapy Management and Disease Management via phone, documenting medical histories, and guiding beneficiaries on treatments. Requires nursing license and bicultural bilingual skills.

Qualifications

• Associate or Bachelor’s degree in Science in Nursing.
• Current and active Nursing License.
• Possess 3+ years of work-related experience.
• Demonstrate knowledge of Therapy Management concepts such as Asthma, Diabetes, Hypertension, CHF, Cholesterol, and others.
• Exhibit excellent oral and written communication skills in Spanish and English (Full Bilingual).

Responsabilités

• Carrying out Medication Therapy Management (MTM) and Disease Management (DM) programs through direct telephone contact with beneficiaries.
• Discussing chronic conditions and medication treatments with beneficiaries to assess health impacts.
• Providing beneficiaries with information on managing their condition(s) and informing them about therapy options.
• Collecting and documenting medical history, medications, lifestyle choices, and other relevant data.

Typical day as Associate Case Manager. You’ll be:

• Carrying out Medication Therapy Management (MTM) and Disease Management (DM) programs through direct telephone contact with beneficiaries
• Discussing chronic conditions and medication treatments with beneficiaries to assess their health impacts
• Providing beneficiaries with important information on managing their condition(s) and informing them about alternative drug therapies and treatments
• Collecting and documenting medical history, medications, lifestyle choices, and other relevant data

Requirements:

• Associate or Bachelor’s degree in Science in Nursing
• Current and active Nursing License
• Possess 3+ years of work-related experience
• Demonstrate knowledge of Therapy Management concepts such as Asthma, Diabetes, Hypertension, CHF, Cholesterol, and others
• Exhibit excellent oral and written communication skills in Spanish and English ( Full Bilingual)
• Can accommodate a flexible hybrid work model with certain on-site days in Puerto Rico
• Are available for on-call hours, including evenings, weekends, and holidays, as operational needs arise

Preferred:

• Have experience working with the geriatric population
• Understand pharmacy benefit manager Clinical Operations and Medicare Part D
