Job summary

Moments Hospice in Saint Cloud, MN is seeking an experienced RN Case Manager to coordinate compassionate end-of-life care for patients at home or in facilities. You will conduct initial assessments, develop and refine care plans, educate patients and families, and collaborate with an interdisciplinary team.

Qualifications

Licensed RN in the state you work in.
Proficiency with EMR software.
Excellent planning, organizing, and communication skills.
Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

Conduct initial client assessments and continuously refine care plans.
Provide hands-on nursing care and patient education.
Maintain open lines of communication with patients, families, and the healthcare team.
Manage patient care coordination for your caseload.
Participate in interdisciplinary team meetings to ensure comprehensive care planning.
