Job summary

moments Hospice is seeking an RN Case Manager to deliver compassionate in-home and on-the-road care, coordinating plans and supporting patients and families through the end-of-life journey.

You will collaborate with owners, participate in care planning, and ensure excellence in clinical practice while maintaining reasonable caseloads. A valid RN license and reliable vehicle are required.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
