Qualifications

• Licensed RN in the applicable state.
• Proficient with EMR software and patient records.
• Strong planning, organizing, and communication abilities.
• Ability to work autonomously and solve problems.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and the care team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings for care planning.
