Job summary

ERP International, LLC is seeking a full time Registered Nurse (RN) Case Manager to support the 1st Special Operations Medical Group at Hurlburt Field, FL. The role focuses on coordination of care, assessment, planning, implementation, and follow-up to ensure continuity of care for military members and their families.

Normal schedule is Monday – Friday, 9 hours between 7:00am and 5:00pm, with a one hour lunch.

Qualifications

Education: Associate Degree of Nursing from NLNAC/CCNE-accredited school.
Certification: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC.
Experience: 1 year as an RN Case Manager post-graduation.
License: active RN license in any state.
BLS: current Basic Life Support certification.
Security: ability to pass a government background check.

Responsibilities

Participate in all phases of the Case Management Program.
Provide nursing expertise on assessment, planning, implementation, and monitoring.
Develop and implement CM strategies across inpatient, outpatient, and telephonic care.
Create tools to support case management and documentation.
Coordinate CM with UM and social work for integrated care.
Maintain liaison with community agencies.
Document patient care data accurately.
Develop treatment plans for preventive, therapeutic, and rehabilitative care.
Ensure follow-up after discharge in ambulatory settings.
Educate patients and caregivers based on identified learning needs.
