Job summary

Vantage Search Group seeks a Registered Nurse - Case Manager at Hurlburt Field near Fort Walton Beach, FL. You will participate in all phases of the Case Management Program, applying nursing expertise to assessment, planning and monitoring of care.

You will develop CM strategies across inpatient, outpatient, onsite and telephonic settings, coordinating with UM and social work. A valid RN license, BLS, and 1 year of post-grad CM experience are required.

Qualifications

• Associates or Bachelors in Nursing from NLNAC/CCNE accredited programs.
• A minimum of 1 year of full-time RN post-grad clinical experience as a Case Manager.
• Valid licensure as a Registered Nurse in good standing.
• Current AHA/ARC BLS Healthcare Provider certification.
• Ability to pass a Government background check/security clearance.

Responsibilities

• Participate in all phases of the Case Management Program and ensure CM standards.
• Provide nursing expertise in assessment, planning, implementation, coordination and monitoring.
• Develop and implement CM strategies using inpatient, outpatient, onsite and telephonic services.
• Integrate CM with UM and social work case management.
• Maintain liaison with community agencies and document patient care data.
• Develop treatment plans for preventive, therapeutic, rehabilitative and psychosocial care.
• Ensure proper implementation of treatment plans and follow-up post-discharge.
• Provide health care instruction to patients and caregivers.
