Job summary

The RN Case Manager, Long Term Care (LTC) at Good Samaritan in Luverne, MN, assesses, implements and coordinates a plan of care for residents, ensuring medications and treatments are administered and physicians are notified as needed.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program, including CCNE/ACEN/NLN CNEA.
• Minimum of two years’ clinical experience preferred.
• Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
• Obtains and subsequently maintains required department specific competencies and certifications.

Responsibilities

• The RN Case Manager assesses, implements, and coordinates a plan of care for residents in pertinent areas.
• Ensures medications and treatments are administered to assigned residents and notifies physicians as needed.
• Directs and assists with resident, family, and significant other education in conjunction with care team.
• Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data.
