Job summary

• Assess referred patients to determine physical, mental, financial, psychosocial status and develop a patient-centered care plan.
• Collaborate with care teams and PCP to implement the plan and coordinate referrals.
• Direct and guide the plan of care to ensure seamless continuum of care; facilitate referrals for home health, LTC, hospice and other services.
• Collect data and provide input to leadership on utilization, cost, quality and care delivery issues.
