Job summary

Qualifications

• Minimum 3 years of experience in relevant clinical area; 5 years hospital case management preferred.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs.
