Job summary

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• Must have one year of experience with a frail or elderly population.
• Must possess a current State of Michigan driver’s license.

Responsibilities

• Assess participants’ needs and plans for appropriate nursing care.
• Collaborate with participant, family, and multidisciplinary team to develop care plan.
• Provide education to participants and caregivers using adult-learning principles.
• Administer medications and treatments as ordered and monitor responses.
• Lead and supervise licensed and non-professional staff in home-based nursing care.
• Document outcomes and revisions to care plans per CSI policies.
