The Waters Senior Living Management LLC in Rochester Township, MN is seeking an RN Case Manager to support the Director of Health and Wellbeing and oversee day-to-day clinical operations, ensuring compassionate, resident-centered care in line with The Waters Way.

You will coordinate nursing processes, partner with healthcare providers, and supervise team members while maintaining data integrity and responding to emergencies.

Qualifikationen

• RN license in good standing in the state where the community is located.
• Preferred 2 years’ geriatric nursing experience.
• Dementia certification preferred or willingness to obtain.
• Previous supervisory and leadership experience preferred.

• Plan, organize, and coordinate the nursing process to provide resident care.
• Collaborate with providers and therapies to align care plans.
• Identify clinical education needs beyond routine requirements.
• Conduct comprehensive clinical assessments for person-centered care.
• Oversee delegation of nursing tasks to team members.
• Maintain clinical data integrity and documentation standards.
• Respond promptly to emergencies and safety incidents.
