Job summary

Vitality Living of New York Mills is hiring an RN Case Manager to join our team. This full-time role works Monday–Friday, 8:00 AM–4:00 PM, delivering resident-centered care and leading the clinical team to ensure quality services comply with licensure requirements.

The RN Case Manager collaborates with residents, families, and staff to assess needs, develop care plans, document services for reimbursement, and provide education and leadership to the team.

Qualifications

• Current, unencumbered Minnesota RN license.
• 3+ years of experience in assisted living or senior care preferred.
• Proficient with electronic medical records and Microsoft programs.
• Tactful and professional communication with residents, families, and team.

Responsibilities

• Conduct initial, routine, and change-in-status assessments.

Vitality Living of New York Mills is seeking a dedicated RN Case Manager to join our team!

The RN Case Manager works collaboratively with the interdisciplinary team to ensure quality care is delivered to our residents based on their individual needs. This role provides clinical leadership, management, education, training, and support to the clinical team while helping ensure compliance with current Assisted Living Licensure requirements.

Key Responsibilities

• Conduct initial, routine, and change-in-status assessments as required.
• Develop, maintain, and monitor resident care plans and services.
• Ensure services are appropriately documented and submitted for reimbursement.
• Provide leadership, education, training, and competency support to the clinical team.
• Support quality care and regulatory compliance throughout the community.
• Work collaboratively with residents, families, staff, and the interdisciplinary team.
