Merakey is seeking a Registered Nurse Case Manager in Warminster, PA. This in-office role includes afternoon travel to group homes across Bucks, Montgomery, and Philadelphia counties.

You will perform clinical assessments, administer medications, and collaborate with a multidisciplinary team to ensure comprehensive care. The position offers a stable weekday schedule and competitive hourly pay of $43 with a $2 enhanced premium.

Qualifications

• Graduation from an accredited Registered Nurse program.
• Current license as an RN in the Commonwealth of PA.
• Preferably, at least one year of experience as an RN.
• Genuine interest in working with individuals with disabilities.
• Possession of a PA driver's license with reliable transportation.

Responsibilities

• Administer medications and treatments with precision and compassion.
• Perform nursing assessments on residents to identify health issues.
• Collaborate with healthcare professionals to coordinate care.
• Provide health education to residents and families, including self-medication programs.
• Conduct in-service training for staff to ensure protocol compliance.
• Communicate health issues with the interdisciplinary team; participate in staff meetings.
• Complete quarterly reports with accurate documentation.
• Travel to group homes in Bucks, Montgomery and Philadelphia Counties.
