Merakey in Warminster, PA is seeking a Registered Nurse Case Manager to join our team. In this role you will provide direct nursing care to individuals with developmental and physical disabilities, perform clinical assessments, administer medications, and collaborate with a multidisciplinary team to ensure comprehensive care.

The position is in-office with afternoon travel to group homes across Bucks, Montgomery, and Philadelphia Counties.

Responsibilities

• Administer medications and treatments with precision and compassion.
• Perform nursing assessments on residents as needed to identify and address health issues promptly.
• Collaborate with healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
• Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
• Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
• Communicate health issues with the interdisciplinary team, actively participating in staff meetings.
• Perform other duties as assigned, contributing to the smooth operation of our program.
• Travel to group homes in Bucks and Montgomery Counties.
