Job summary

Fallon Health is seeking an experienced Assessment Nurse Case Manager in Massachusetts to perform home visits for onboarding, completing HRAs and MDS assessments. The role involves in-person and telephonic assessments, collaboration with the care team, and ongoing education of members about NaviCare case management.

Qualified candidates are RNs with MA licensure, 1+ year of complex care experience, and a willingness to work in a dynamic, member-centered environment. Driving is required.

Qualifications

• Graduate from an accredited school of nursing is mandatory.
• A Bachelor’s (or advanced) degree in nursing or healthcare-related field preferred.
• Active MA Registered Nurse license required.
• Certification in Case Management strongly desired.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Conducts in-home visits for onboarding and regulatory assessments.
• Completes HRAs, PCA assessments, and MDS submissions.
• Provides education on NaviCare case management program.
• Performs telephonic/virtual assessments when appropriate.
• Collaborates with the care team and UM.
• Participates in training and audits.
