Job summary

Great Falls Clinic Hospital is seeking a Registered Nurse Case Manager to optimize resource use and coordinate patient care across disciplines. You will align services with patient needs, ensure compliance with case management standards, and work with a multidisciplinary team to support efficient discharge planning.

The role requires MT RN licensure and BLS, with CCM/ACM/CMCG preferred. A strong emphasis on leadership, collaboration and patient-centered care defines this opportunity.

Qualifications

• Graduate of an accredited nursing program and licensed as an RN in MT.
• Minimum 2 years healthcare experience as an acute care case manager or equivalent.
• Preferred: 3 years acute care case management and Utilization Review experience.
• Licenses: MT RN license or compact, BLS; CCM/ACM/CMCG preferred.

Responsibilities

• Promotes cost effective allocation of resources while maintaining high quality care.
• Coordinates patient care, treatment and services based on patient needs.
• Complies with accrediting body standards for Case Management/Discharge Planning and Utilization Review.
• Works cooperatively with Case Management staff, providers, UR Committee Members and leadership.
