Qualifications

• RN licensure required; ADN minimum.
• BSN preferred; case management experience preferred.
• Direct patient care experience preferred.
• Strong communication and problem-solving skills.

Responsibilities

• Coordinate and manage care for patients with chronic or complex conditions.
• Conduct comprehensive patient assessments to identify needs.
• Develop and evaluate individualized care plans with interdisciplinary teams.
• Monitor progress and adjust care plans as needed.
• Advocate for residents' needs and preferences throughout care.
• Facilitate communication among patients, families, and providers.
• Arrange appointments, referrals, transportation, and community resources.
• Ensure efficient use of resources while maintaining high-quality care.
• Educate patients and families about diagnoses and options.
