Cape Cod Post Acute is seeking an RN to deliver skilled nursing care and monitor resident conditions in a 135-bed Sub-Acute and Long-Term Care facility. The role emphasizes adhering to individualized care plans and collaborating with the interdisciplinary team.

Qualifications

• Current, unencumbered MA RN license.
• Valid BLS (CPR) certification required.
• Ability to manage care for multiple residents in a fast-paced setting.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide resident care per physician plan and written care plan.
• Follow and implement care plans by Nurse Manager.
• Administer medications per plan of care (POC).
• Monitor residents and report changes to the clinical team.
