Cape Cod Post Acute and Healthcare Center is seeking an RN Case Manager to oversee care coordination, support clinical decision-making, and ensure continuity of care for residents. You will conduct assessments, develop care plans, monitor outcomes, and educate patients and families, while coordinating care across interdisciplinary teams.

Qualifications

• RN with ADN required; BSN preferred.
• Strong communication, negotiation, and problem-solving skills.
• Understanding of healthcare regulations and care coordination principles.

Responsibilities

• Coordinate and manage care for patients with chronic, acute, or complex conditions.
• Conduct comprehensive patient assessments to identify needs.
• Develop and evaluate individualized care plans with interdisciplinary teams.
• Monitor progress and adjust plans based on outcomes.
• Advocate for residents and communicate needs and preferences.
• Facilitate communication among patients, families, physicians, and providers.
• Coordinate appointments, referrals, transportation, and community resources.
• Ensure efficient use of resources while maintaining quality care.
• Educate patients and families about diagnoses, options, and planning.
