Qualifications

• RN required in Tennessee
• 1-3 years Case Management experience
• 1-3 years clinical healthcare experience (hospital)
• Strong collaboration with interdisciplinary team
• Knowledge of transitions of care and discharge planning

Responsibilities

• Coordinates the delivery of clinical services to ensure patient outcomes and cost-effectiveness goals.
• Performs initial assessment to identify readmission risk and plan transitions.
• Communicates with physicians, care team, patients and families; educates patients for informed decisions.
• Plans care activities to meet patient needs and manages length of stay and资源 utilization.
• Advocates for safe discharge to home or community resources and supports follow-up.
• Maintains accurate documentation and tracking of cases per standards.
• Performs additional duties to support department and organizational goals.
