Job summary

Qualifications

Responsibilities

• Facilitate care transitions through effective resource management and assessment of patient needs.
• Balance clinical and financial considerations to advocate for patient needs while managing resources prudently.
• Develop and implement transition plans based on patient assessment, preferences, and available resources.
• Identify at-risk patients for readmission, perform risk assessments, and coordinate referrals to Social Work or Complex Case Review.
• Screen patients for factors affecting care progression and intervene to promote timely throughput.
• Ensure care is provided at the appropriate level based on medical necessity, preventing avoidable readmissions.
