Job summary

Northshore Home Health Care is seeking a Registered Nurse Case Manager to provide comprehensive home-based care for patients with complex needs in Massachusetts.

You will develop and evaluate individualized care plans, coordinate with physicians and health teams, educate patients and families, and ensure quality outcomes while traveling locally to patient homes.

Qualifications

• Current RN license in the Commonwealth of MA.
• BSN preferred.
• Minimum of three years, ideally five, of home care clinical experience.
• Proficient with Microsoft Office and case management data systems.
• Ability to travel locally; Spanish speaking skills are a plus.
