Job summary

Qualifications

• RN licensed with unrestricted license in relevant state
• Successful completion of case management orientation program
• Minimum of 2 years full-time acute inpatient setting or specialty area
• Excellent communication skills and basic computer/typing ability

Responsibilities

• Develop care plans to meet patient health care needs
• Act as patient advocate and coordinate care with providers
