• Actively participates with clinic redesign and PCMH expansion.
• Collaborates with primary care staff, social work, and other care team members to ensure optimal care coordination.
• Initiates communication with high‑risk/targeted population patients.
• Pulls data from multiple systems to create a complete performance picture for targeted populations.
• Implements interventions based on data and guidelines and monitors data for trends.
• Optimizes patient care transitions with appropriate post‑acute plans and discharge planning.
• Guides patients through the health care system and provides disease/treatment education to patients and families.
