Job summary

Bethesda Corporate; Home Health is seeking a Registered Nurse to organize, coordinate and direct services for patients in the agency. You will ensure quality care, supervise team members when needed, and maintain open communication with patients, families and physicians to promote positive outcomes.

You will develop and update the plan of care, document in the EMR at point of care, complete physician summaries at SOC/Recert/Transfer/ROC, and maintain accurate visit schedules while participating

Qualifications

• Registered nurse licensed in Missouri in good standing.
• Bachelor's and/or Master's degree preferred in nursing.
• Minimum two years of professional nursing experience; home health/hospice preferred.
• Strong written and oral communication; solid documentation skills.
• Ability to lift/move 10–200+ lbs with proper technique and assistive help.

Responsibilities

• Organizes, coordinates and directs ongoing provision of services to assigned patients.
• Ensures delivery of quality patient care and supervises staff as needed.
• Initiates and maintains communication with patients, families, physicians and team members.
• Develops and updates the plan of care with patient and care team.
• Documents into EMR at point of care to keep up-to-date records and accuracy.
• Completes required physician summaries at SOC, Recert, Transfer, ROC and discharge.
• Maintains an accurate visit schedule and participates in on-call schedule and QA/QAPI activities.
• Familiar with agency policies and regulatory rules; may take on additional duties as appropriate.
