Job summary

Pathways Health Partners seeks a Case Manager – Nurse (RN) to round on hospitalized patients, coordinate care with bedside staff and treating providers, and assess post-discharge needs. The role includes documenting assessments for post-acute care across inpatient, ER and observation units.

The ideal candidate holds an RN license in Florida (BSN preferred) with 2+ years in case management. CPR/BLS/ABLS certification at hire or within 30 days is required.

Job Summary

Primary duties of the Case Manager – Nurse (RN) will include rounding patients in the hospital, communicating with patients at bedside and other treating providers for care coordination, identify post discharge needs and barriers to care and provides resources for patients. The Case Manager – Nurse (RN) will conduct and document assessments of patient needs and resources for effective post-acute care needs. The assessments take place throughout the hospital including in-patient, emergency room and observation units

Role and Responsibilities

• 1. Regularly utilize science knowledge obtained during RN program to assist in the patient’s medical condition(s) treatment plan
• 2. Maintain documentation standards in required systems (EHR)
• 3. Conduct thorough assessments of patients' health status, including physical, psychosocial, and functional needs based on bedside interview with patient, family and the review of medical records.
• 4. Know and maintain professional communication skills: verbal, written, and non-verbal communication
• 5. Understand and maintains the highest level of HIPAA (Health Insurance Portability and Accountability) standards

Qualifications/Position Requirements:

• 1. Education: Successful completion of a Registered Nurse program from an accredited college, BSN preferred; or a combination of education and relevant experience.
• 2. Experience: Minimum 2-year practicing RN Case Manager preferred
• 3. Certification/Licensure: Active and Clear RN License in the state of Florida. CPR/AED/BLS certified at time of hire or within 30 days
• 4. Training and Knowledge Required at Time of Hire: Registered Nurse program curriculum completion
