Job summary

Ochsner Health is seeking a Registered Nurse to manage pre-admission, admission review, and discharge planning during hospitalization. You will collaborate with ED physicians, hospitalists, and care teams to determine medically necessary care levels and coordinate discharge planning with patient-focused communication.

The role requires an active RN license, BLS through the American Heart Association, and experience with discharge planning; a Bachelor’s degree is preferred.

Qualifications

• Graduate of an accredited school of nursing is required.
• Bachelor’s degree in nursing preferred.
• Current RN license in state of practice is required; BLS from AHA is required; CCM/ACM preferred.

Responsibilities

• Perform pre-admission, admission, and continued stay reviews per CMS and payer guidelines.
• Coordinate discharge planning and communication among ED physicians, hospitalists, attending physicians, and care teams.
• Maintain knowledge of participation requirements and ensure documentation in medical records.
