Ochsner Health in Louisiana is seeking a nurse for discharge planning and utilization review to optimize patient care during hospitalization. You will review admissions, coordinate level of care, and collaborate with ED physicians, hospitalists, and payers while ensuring accurate documentation.

The role requires nursing licensure in the state, BLS, and a willingness to work a flexible schedule, including nights and weekends.

Qualificações

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Current registered nurse license in state of practice.
• Basic Life Support (BLS) certification from the American Heart Association.
• CCM or ACM certification preferred.

Responsabilidades

• Performs pre-admission, admission, and continued stay reviews under CMS and payer guidelines.
• Maintains open communication with ED physicians, hospitalists, attending physicians, and physician advisors.
• Leads the oversight and management of discharge planning during the acute-care hospitalization.
• Maintains knowledge of conditions of participation related to utilization review and discharge planning.
• Communicates with patient, caregivers, and multi-disciplinary team throughout the patient care process.
• Ensures documentation in the medical record and compliance with policies.
