Job summary

Ochsner Health in Louisiana seeks a registered nurse to oversee discharge planning and utilization review across the hospital stay. You will review hospitalizations, recommend appropriate levels of care, and coordinate with ED physicians, hospitalists, and payer teams to ensure compliant, patient-centered discharge plans.

Required is a nursing degree with active RN license; BLS is mandatory. Preference for 3 years in discharge planning or case management and CCM/ACM certifications.

Responsibilities

• Performs pre-admission, admission, and continued stay reviews per CMS and payer guidelines.
• Liaises with ED physicians, hospitalists, and care teams to coordinate discharge plans.
• Maintains knowledge of participation requirements related to utilization review and discharge planning.
• Communicates with patients, families, and payers to facilitate appropriate level of care.
• Leads discharge planning oversight to optimize functional and financial outcomes.
• Adapts communication style to diverse patient populations and care settings.
• Ensures compliance with laws, regulations, and standards.
