Job summary

Moments Hospice seeks an RN Case Manager to coordinate high-quality patient care across final days, weeks, and months. You will work closely with the owners and participate in entrepreneurial growth while managing a caseload with support from an experienced team.

Qualifications

• RN license in the state you work in (Wisconsin)
• Proficiency with EMR software
• Excellent planning, organizing and communication skills
• Ability to solve problems autonomously while maintaining high standards
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Conduct initial assessments and refine care plans for each client
• Provide hands-on nursing care and patient education
• Maintain open communication with patients, families, and healthcare team
• Coordinate patient care for your caseload
• Participate in interdisciplinary team meetings to ensure comprehensive care planning
