Job summary

Moments Hospice is seeking an experienced RN Case Manager to help make patients' final days as comfortable as possible. You will conduct assessments, refine care plans, provide hands‑on nursing care, and coordinate with families and the healthcare team.

Work closely with the owners and participate in our entrepreneurial spirit, fostering internal growth and clinical advancement. Expect a predictable caseload and on‑road nursing with on‑site support as needed.

Qualifications

• Licensed RN in the relevant state (WI).
• Proficiency with EMR systems.
• Excellent planning and communication skills.
• Ability to work autonomously while meeting standards.
• Valid driver's license and reliable vehicle with insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the health care team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings for care planning.
