Job summary

Assisted Home Health & Hospice in Ventura, CA is seeking a dedicated RN Case Manager (Hospice) to join our team. This full-time position requires Spanish language skills and close-to-home scheduling to support our hospice patients in the Ventura area.

The RN will complete initial and ongoing patient assessments, provide skilled nursing care, and coordinate services to meet patient needs. A CA RN license, CPR, and TB clearance are required, with opportunities for growth within our supportive

• Current, active California RN license.
• Current CPR, health clearance, TB clearance.

Responsibilities

• Have flexible hours and work close to home.
• Complete initial and ongoing comprehensive assessments of patients' needs.
• Provide nursing services requiring substantial and specialized nursing skills.
• Initiate preventative and rehabilitative nursing procedures.
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
