Job summary

Assisted Hospice is seeking a per diem RN Case Manager in Mission Hills, CA to perform 10-15 visits weekly plus on-call shifts. The role offers per-visit pay with additional mileage fees and on-call rates for evenings and weekends, rewarding patient care across our Mission Hills branch.

Ideal candidates are graduate nurses with CA RN licensure, CPR certification, and flexible availability to serve patients in the community.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of California.
• Current CPR certification.

Responsibilities

• Have flexible hours and work close to home (reduce your commute).
• Complete initial and ongoing comprehensive assessments of the patient's needs.
• Provide service and/or treatments requiring substantial nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.

Assisted Hospice is looking for a per diem RN Case Manager for our Mission Hills branch to pick up 10-15 visits per week plus some on-call shifts (nights and weekends are available).

Please note, we pay per visit. See rate of pay below.

Regular visit rates

• Admission- $200 per visit + $11 for mileage
• Routine- $70 per visit + $11 for mileage

On-call rates

• Weekday waiting to engage rate- $50 per day
• Weekend waiting to engage rate- $100 per day
• Admission- $200 per visit + $11 for mileage
• Routine- $105 per visit + $11 for mileage
