Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate end-of-life care on the road. You will assess patients, develop and refine care plans, and collaborate with families and the health team to ensure comfort and dignity.

The role includes hands-on nursing care, patient education, and coordinating care across your caseload. A valid RN license, driving required, and EMR proficiency are essential for success.

Qualifications

• Licensed RN in the state you work in.
• Proficient with EMR software.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle with insurance.
• Ability to solve problems autonomously.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
