We are hiring a Registered Nurse Case Manager to join our team. $3500 Sign-On Bonus offered.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Conduct initial nursing evaluations within 48 hours of referral to determine eligibility.
• Identify and reassess the patient's physical, psychosocial, and environmental needs at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with the patient/family, attending physician, and IDG team daily.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes regarding the patient's condition.

Education and Experience

• Minimum one year of clinical experience.

License Requirements

• Current RN licensure in the state of practice.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Additional State-Specific Requirements

• LA: At least two years of full-time RN experience or two years of hospice care as an LPN substituted for RN experience.
