We are hiring for a Registered Nurse Case Manager position, offering a $3500 Sign On Bonus.

As a Hospice RN, you will:

Responsibilities:

• Assume full nursing responsibility for delivering the Plan of Care for each hospice patient.
• Perform initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
• Identify and reassess the patient's physical, psychosocial, and environmental needs at least every 15 days.
• Coordinate and initiate the plan of care.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with the patient/family, attending physician, and other team members daily.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.

Education and Experience:

• At least one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR Certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

Additional State-Specific Requirements (e.g., Louisiana): Minimum two years of full-time RN experience, or two years of hospice care experience as a licensed practical nurse as a substitute.
