Job summary

ALIGN HOSPICE CARE LLC seeks a Registered Nurse Case Manager to develop personalized care plans, maintain up-to-date patient records, and ensure clear communication among patients, families, and the care team. You will serve as the primary contact for patient care-related matters.

Qualifications

• Current Registered Nurse (RN) license in the state of employment.
• Experience as an RN case manager is preferred.
• Excellent interpersonal and client care skills.
• Valid driver's license.
• First aid/CPR certification required.

Job Summary

We are seeking a Registered Nurse Case Manager to join our hospice team. In this role, you will formulate the best care plan for each individual patient assigned to you, make sure patients records are up to date and maintain clear, consistent communication between the patient, the patient's family and the patient's care team as you will be the point of contact for most things patient care related.

You will be a good fit for this job if you are a RN who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
