Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for an outpatient role in the Plattsburg area. The RN will admit and coordinate care, perform admissions and assessments, and manage discharges and pronouncements in collaboration with the care team.

Qualifications include Missouri RN licensure and preferred 1+ years of RN experience, with hospice or home health background. Strong communication and travel ability are required; a clean background and driving record are mandatory.

Qualifications

• Licensed RN in Missouri required.
• 1+ years RN experience preferred.
• Experience with Hospice or Home Health preferred.
• Effective written and verbal communication.
• Ability to travel; reliable transportation required.
• Clean background, drug screen, and driving record.

Responsibilities

• Admit and coordinate appropriate care for each patient and family.
• Perform admissions, assessments, discharges and pronouncements.
• Collaborate with the interdisciplinary care team to ensure care continuity.
