Job summary

Altus Hospice Care is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our devoted nursing team delivering exceptional end-of-life care throughout the Temple community. This per diem role supports patients and families with dignity, peace, and personalized care.

You will empower with clinical referrals, collaborate with the Clinical Director to assess patient eligibility, and provide direct care focused on comfort and quality of life in a supportive, mission-driven

Qualifications

• Hold a current unencumbered RN license.
• Minimum of 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification if required by state.
• Must have a valid driver’s license and transportation.
• Proficient with computer programs and EMR systems.

Responsibilities

• Respond to clinical referral information with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct, compassionate nursing care to patients to enhance comfort and quality of life.
• Deliver nursing care rooted in established principles and techniques.
• Educate patients and families to support active participation in care plans.
• Share in call duties to ensure timely availability and support.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Pay

$36.00 - $42.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
