Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients and families in a field-based role, coordinating services and ensuring comfort at end of life. The position offers a competitive salary and a fleet car program for business and personal use.

The role requires an active RN license, EMR proficiency, excellent organizational and communication abilities, and the capacity to work independently in a compassionate, patient-centered environment.

Qualifications

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
