Job summary

Harbor Healthcare System in the Lufkin area seeks a qualified full-time Registered Nurse Case Manager. This outpatient role requires an independent, experienced RN to coordinate admissions, assessments, discharges and pronouncements as part of the interdisciplinary care team.

Responsibilities include patient and family coordination, travel as needed, and ensuring smooth transitions of care. Texas RN license, 1+ year RN experience, and strong communication are required.

Qualifications

• Texas RN license required.
• 1+ year RN experience preferred.
• Hospice or Home Health experience preferred.
• Excellent written and verbal communication.
• Must be able to travel and have reliable transportation.
• Clean background, drug screen, and driving record.

Job Description

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications

• Licensed RN in the State of Texas - required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
