Job summary

Pathways Hospice is seeking a full-time RN Case Manager to cover the Northwest, Southwest & Southeast Houston areas. You will oversee the nursing care team and coordinate with physicians, therapists, and other professionals to deliver comprehensive home health services.

You will conduct assessments, develop care plans, and educate patients and families on managing health conditions and medications. Night/weekend duties and an on-call rotation are required to meet patient needs.

Qualifications

• Valid RN license in Texas.
• Hospice experience (1 year).
• Knowledge of home health regulations and healthcare compliance.
• Experience with HomeCare HomeBase (HCHB) EMR (preferred).
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals for cohesive patient care.
• Plan, organize, and direct home care services.
• Assess patient care needs and develop care plans.
• Educate patients and families on health management and medications.
• Prepare clinical notes accurately and promptly.
• Night and weekend coverage required per hospice needs.
• On-call rotation weekly.

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly)

Qualifications:

• Valid RN license in the State of Texas.
• Hospice Experience (at least 1 year).
